Why men need different BMI consideration

Men have ~10% lower body fat than women at equivalent BMI. They store fat preferentially around the abdomen (visceral fat), which is more metabolically harmful. BMI often misclassifies muscular men as overweight.

Why: Visceral abdominal fat is the strongest predictor of cardiovascular risk in men. Waist >102 cm (40 in) indicates increased risk regardless of BMI.

How: Measure at narrowest torso point, usually at navel level. Stand relaxed, breathe out normally.

Body fat percentage

Why: Healthy male body fat: 10–22% (varies by age and athletic status). Below 6% is too low; above 25% indicates obesity regardless of BMI.

How: Skinfold calipers (cheap, ±3% error), bioelectrical impedance (convenient, ±5% error), DEXA scan (gold standard, $100–300).

FFMI (Fat-Free Mass Index)

Why: Specifically designed for muscular men. FFMI 20–22 is athletic; 22–25 highly muscular; above 25 suggests above-average genetics or training.

Myth

All men with BMI 27+ are overweight

FFMI studies show ~30% of men classified as overweight by BMI have healthy body-fat percentages. This is especially common in regular exercisers and naturally muscular builds.

Myth

Men's BMI ranges should be higher than women's

WHO uses identical numerical ranges for both sexes. The interpretation differs (men have less fat at same BMI), but the cutoffs themselves are not sex-specific.

Athletes & bodybuilders

Muscle weighs more than fat. A heavily-muscled person may have a BMI in the 'overweight' range while having very low body fat. Use FFMI instead.

Pregnant women

BMI is not validated during pregnancy. Use pre-pregnancy BMI for reference or talk to your OB/GYN.

Older adults (65+)

Older adults often have healthy weights at BMI 23–30. Underweight is more dangerous than slight overweight at this age.

Asian populations

Health risks emerge at lower BMI values. The WHO recommends 23 as the overweight cutoff (vs 25 in the general standard) — Singapore, China, India, Japan and the Philippines apply adjusted ranges.
